CMSA Photo IDs

If you do not have a CMSA Photo ID Card or need to replace your old Card, please follow these instructions:

Step 1. Get CMSA ID number.
To obtain your CMSA ID number from NorthStar Soccer please fill in your home phone number in the form below and submit the form. Also you can now try CMSA's Look Page: Click Here. You will be emailed your CMSA ID. If you do not get a number, or your number is zero, please continue to follow the steps below and set up an appointment (use a fake number to book the appointment). Please contact the Registrar if you are experiencing difficulty: Registrar@NorthStarSoccer.ca.

Step 2. Schedule CMSA Photo ID session ($10 fee).

To book an appointment with CMSA click here. You cannot just show up without an appointment.

Remember to take:
- proof of id and age
- your CMSA ID number
- $10 cash for a new or lost card, (if you are exchanging an old card, there is no charge so long as you trade in the old card).
